Isosorbide dinitrate
Indications/Uses
Severe or unstable angina pectoris. Unresponsive left ventricular of various etiology & failure secondary to acute MI. During percutaneous transluminal coronary angioplasty to facilitate prolongation of balloon inflation. Prevent or relieve coronary spasm.
Dosage/Direction for Use
Adult IV 2-12 mg/hr; may adjust up to 20 mg/hr depending on patient's response. Intracoronary 1 mg as a bolus inj prior to balloon inflation. Max: 5 mg w/in 30-min period.
Contraindications
Hypersensitivity to isosorbide dinitrate, to other nitrates or nitrites. Low filling pressure, hypertrophic obstructive cardiomyopathy, constrictive pericarditis, cardiac tamponade, cardiogenic shock, circulatory collapse, aortic &/or mitral valve stenosis, severe hypotension (systolic BP <90 mmHg), head trauma & cerebral hemorrhage, increased ICP-associated diseases, marked anemia, hypovolemia, closed-angle glaucoma. Use of phosphodiesterase inhibitors (eg, sildenafil, tadalafil, vardenafil).
Special Precautions
Patients w/ hypothyroidism, hypothermia, malnutrition, severe liver or renal disease, orthostatic syndrome. Development of tolerance; hypoxia particularly in patients w/ CAD leading to myocardial hypoxia; patients on controlled Na diet. Monitor BP & pulse rate. May affect ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Headache, dizziness, somnolence, tachycardia, aggravated angina pectoris; orthostatic hypotension, flushing, collapse, severe hypotensive responses (including nausea, vomiting, restlessness, pallor & excessive perspiration), hypoxia, heartburn, allergic skin reaction (eg, rash), angioedema, SJS, exfoliative dermatitis, feeling of weakness.
Drug Interactions
Potentiates hypotensive effects w/ phosphodiesterase-5 inhibitors eg, sildenafil; BP-lowering drugs (eg, β-blockers, Ca antagonists, vasodilators) &/or alcohol; neuroleptics & TCA(s). May increase blood level of dihydroergotamine & its antihypertensive effect. Use w/ riociguat can cause hypotension.
